Endo Belly: Why Endometriosis Can Cause Severe Bloating

Many people living with endometriosis experience sudden and severe abdominal bloating often referred to as “endo belly.” This term describes the painful abdominal swelling that can occur before or during menstruation and can make the abdomen appear noticeably distended.

Endo belly is different from typical bloating. For some people, it can appear quickly, may be associated with specific times of the menstrual cycle, and be accompanied by pelvic pain, digestive discomfort, fatigue, or changes in bowel movements.

Why Endometriosis Can Cause Bloating

Several factors may contribute to endo belly.

Inflammation

Endometriosis is associated with chronic inflammation in the pelvic cavity, which can lead to swelling and irritation of surrounding tissues. The buildup of endometriosis lesions can cause further inflammation and water retention. 

Digestive system involvement

Endometriosis can affect structures near the bowel, which may lead to bloating, constipation, gas, or painful bowel movements. It is also associated with alterations to the gut microbiome which can further the bloating. 

Hormonal fluctuations

Changes in estrogen and progesterone throughout the menstrual cycle can also influence digestion and fluid retention. Both of these hormones play a role in endometriosis symptoms and progression. 

When Bloating Might Be a Sign of Endometriosis

Bloating can be common, but it may be worth investigating further if it:

  • worsens around your menstrual cycle

  • occurs with painful periods, pelvic pain, or other symptoms of endo

  • is accompanied by pelvic pain or pain during sex

  • occurs alongside bowel symptoms during menstruation

Digestive symptoms are sometimes mistaken for gastrointestinal conditions before endometriosis is even considered.
